QUESTION 1:

1.PULMONOLOGY

                      Insights: I find that all the symptomatology is well explained clearly. This helps in identification of particular problem. Etiology and anatomical location of the problem is well identified and mentioned.

Insights:- Efficacy of each drug was well mentioned.

evolution of symptomology well explained. it was well comprehensable, easy to understand.

3) CARDIOLOGY

           INSIGHT -The symptomatology, anatomical localization are briefly mentioned. Primary etiology is explained.

            All the administered drugs and angioplasty are clearly explained. Overall the document was written in such a way that it is easy to understand.

4) INFECTIOUS DISEASE

INSIGHTS -PRIMARY ETIOLOGY- IMMUNO SUPRESSION by DIABETES 

• diabetes is main drawback for mucomycrosis.

•  amphoterin b effective against most invasive fungal infections.

Insights :- Efficacy of drugs well explained and all the investigations were perfomed.

                     Insights: Various pharmacological actions over the placebo is explained. Importance of nebulization in pharmacological and non pharmacological interventions is well described.

7. Case

                  Insights: The reason for the dyspnea and its relation to pancreatitis are valid. Reasons are good enough to prove the clinical symptoms.

8.RHEUMATOLOGY

INSIGHT

This is a case of COVID-19 Pneumonia in a preexisting case of interstitial lung disease.

 The etiology was very intellectually presented and the clearly described the usage of the drug clexane.

9) CARDIOLOGY

         INSIGHT-The evaluation of symptomotology of the patient is done nicely. They were arranged in chronological order. Anatomical localization is mentioned. Primary etiology is also explained.

          Mechanism of action of drugs, indication and efficiency are given along with graphs and references. Renal involvement in the heart failure is also explained clearly. Other answers were also written.

                   Insights: Every indication and contraindication are well mentioned including reasoning regarding ST elevation.

This yet has to be done. I’ve been seeking help from my classmate on the learning of the her ongoing case, soon after which i’ll be doing one of my own. Once it is completee i will update it here.

            A 70 year old patient presented withdistended abdomen and shortness of breath.She is a hypothyroid patient since 5 years.so she was on thyronorm 100mg .Her apetite was normal with diet mixed regular bowel and bladder movements sleep is also adequate.There is slight variation in temperature all other vitals are normal.Syatemic examination is also done accordingly.INVESTIGATION done are doneECG DONE AND SHOWN ATRIAL FIBRILLATION

                      

TREATMENT

Inj. Amiodarone 150 mgIV stat (2 doses)

Inj.Amiodarone infusion

 1mg/min till 6hr f/b 0.5 mg/min for next 18 hours

 Inj.clexane 40mg Sc OD                                                                                                                    

Treatement plan is also nice but they failed to control the thyroid levels in this patient if they have done so she would have not passed away during defibrillation.             

QUESTION 5

we were all very excited and  enthusiastic that we could enter into a very intresting and a new Hospital  environment where we could come across a wide range of cases and daily there  would be some or other sort of learning  but Due to this pandemic we are unable to appear directly and experience the patient but even though we had a very good experience.that was possible only because of the gen medicine department .
